Just last week, he was named in the Principality Premiership Team of the Year for 2012/13.
Head coach Neil Edwards explained his choice of appointing Evans as his side's leader.
"As a player Lee works as hard as he can constantly," Edwards said.
"That quality is mirrored in his personality.
"Lee is a natural leader, he optimises the Neath spirit that will help us grow throughout this new group. He is dedicated to the growth and development of our team and club.
"He has proven his commitment to the club by staying with Neath through a difficult couple of seasons, even though as one of the best players in his position in the Premiership, he could have easily have accepted any number of offers to move on.
"I am proud of Lee for achieving this accolade, it is thoroughly deserved."
